Tour letter dated October 25, 1940, addressed to our Master Mechanic, Mr* Talbert, at Mobile, submitting your resignation from the services of this company as of that date, in order to apply for an annuity under the Railroad Retirement Board Act, was received by Mr. Talbert on January 10, 1941.
According to our records, you were born December 26, 1872, entered our service November* 1895 and last worked on October 24, 1940.
Under this arrangement you are to be permitted to continue group insurance in the amount of #2000.00, by paying the monthly premiums in cash® Please understand that such premiums must be paid in advance; that is remittance must reach our Treasurer in Louisville not later than the last day of the month preceding that to which it applies; the February, 1941, premium, for instance, must reach our Treasurer not later than January 31, 1941.
